What we do
Quality assurance and improvement across your whole service
Clinical Supervision
Robust clinical supervision is a CQC expectation — and a genuine protective factor for your staff and the people they support. We provide structured, reflective supervision for clinical and care staff, and help you build internal supervision frameworks that are sustainable, documented, and evidenced.
Safeguarding Support & CQC Notifications
When things go wrong, the response matters as much as the incident. We support you through safeguarding processes — from initial concern through to investigation, referral, and CQC notification. We help you understand your statutory duties, meet your reporting obligations, and respond in a way that protects people and your registration.
Confidential Investigations & Lessons Learned
We conduct independent, confidential investigations into serious incidents, complaints, and near misses. Our process is thorough, fair, and focused on learning — not blame. We produce clear root cause analysis reports, identify systemic failures, and co-produce action plans that prevent recurrence.
Day-to-Day Care Quality Improvement
Quality is not just for inspection time. We work alongside your team to improve the everyday quality of care — through observation, audit, care planning reviews, and practice development. We help you build the evidence of good practice that tells your story to CQC inspectors.
CQC Inspection Readiness
We help you prepare for CQC inspection with confidence — not anxiety. We conduct mock inspections, review your evidence portfolio, identify gaps, and coach your team on how to present their work. We know what inspectors look for because we have worked inside regulated services.
Engaging with Safeguarding & CQC
Navigating safeguarding partnerships and CQC relationships can feel daunting. We support you to engage constructively — attending strategy meetings, supporting you through Section 42 enquiries, and helping you build a positive, transparent relationship with your local authority and regulator.
How we support you through serious incidents
Incidents happen in every care service. What matters is how you respond. We provide calm, expert support from the moment something goes wrong through to lessons learned — protecting people, your staff, and your registration.
Immediate response support
We help you stabilise the situation — ensuring the right people are notified, immediate risks are managed, and your statutory duties are met from the outset.
Independent investigation
We conduct a thorough, confidential investigation — interviewing staff, reviewing records, and building a clear picture of what happened and why.
Root cause analysis
We go beyond the surface event to identify the systemic and organisational factors that contributed — giving you a genuine understanding of the failure.
Lessons learned action plan
We co-produce a practical, time-bound action plan that addresses root causes, embeds learning, and demonstrates to CQC that you have responded appropriately.
Inspection readiness
Building your evidence of good practice
CQC inspectors want to see evidence — not just hear about it. We help you build a robust, well-organised evidence portfolio that tells the story of your service clearly and confidently.
